A proof of residence is required in Manatee County to enroll all students. You may use one of the following:
1. A current electric or water bill in the parent/guardian's name.
2. A current lease agreement with the parent/guardian's.
3. An official letter from company providing housing with parent/guardian's name.
If a parent cannot provide the above requirements, they must visit the Parent Information Center for verification of address.
Parent must bring:
1) A notarized statement from the owner/lessee of the home where they are living, stating the names of all people residing at the address. In addition, parents must bring a current utility bill (power or water) with the owner/lessee's name and address.
The Parent Information Center has a notary on staff and will be happy to notorize this form for you. A picture ID will be needed for this service.
