BHS Science Fair 2011-2012: Forms

IMPORTANT INFORMATION ABOUT FORMS!

1) Forms must be typed or written in blue or blank ink. All signatures and dates MUST BE WRITTEN IN INK.

2)The Intel website allows you to type on the forms and you can save them to your computer or other external device. This allows you to edit them and print them out at any time.
Form 1 - Adult Sponsor
The Adult Sponsor form is completed by the adult sponsor. The adult sponsor is typically a science teacher. This form helps students and sponsors figure out additional forms that may be required for the project.
Form 1(A) - Student Checklist and Research Plan
If the student is working individually or is part of a team, he or she is to complete this form.

*IMPORTANT INFO ABOUT FORM 1A*
1) Students may work in teams of up to three people. Once the project is approved, additional team members may not be added or replaced by other students. In other words, a team can shrink from 3 to 2 people but a person cannot be "booted" out and replaced by someone else nor can a team of two people add on a third person after they have started the project.
2) If the project requires SRC and/or IRB approval, the project start date CANNOT be prior to SRC/IRB approval. A student who begins before receiving approval will be disqualified.
3) The dates on this form MUST BE WRITTEN IN INK!

THE RESEARCH PLAN
It is essential that participants follow the instructions on this form very carefully. Projects without an acceptable research plan will not be approved.

- Hypothesis: This must be a testable statement.
- Experiment: Identify all variables and make sure that the procedure is very specific. Include how data will be collected (for example, a blank data table).
- Sources: Every project must refer to a minimum of 5 credible sources. The rulebook can be used as a valid source. Wikipedia and non-peer reviewed sources are strongly discouraged.
Form 1(B) - Approval Form
The approval form is signed by the student and her/his parent or guardian. Depending upon the project additional signatures may be required. The adult sponsor will guide the student through the approval process.

Form 3 - Risk Assessment
Using power tools? Does your project require chemicals? Are there any dangers associated with your project? You might need to conduct a risk assessment. Your adult sponsor can assist you.
Form 4 - Human Subjects
If your project requires human subjects, you MUST complete this form. You may also complete an Informed Consent Form (see below). Speak with your adult sponsor about this.
Informed Consent Form
This form may need to be completed if your project involves human subjects. Your adult sponsor will confirm if you need to take this step.
Form 5 - Vertebrate Animals
Working with vertebrate animals is serious business. You MUST complete this form if your project involves animals with backbones.
Form 6A - Biohazards
See the rulebook for a list of biohazardous materials. If your project requires anything on that list, you need to complete this form.
Form 6B - Tissues
This form has nothing to do with Kleenex. If your experiment includes blood, feces, saliva, or any body tissues, then this form needs to be filled out.
